What we call vandalism

For us vandalism is:

edit a user's userpage with nonsense/undesirable comments
edit an official page with nonsense or something not related to the page
insult a user on any' page (even if it's a testpage)
not respecting a policy (after a warning)

Please use common sense for the rest.
