There's only three things left to do!

Just replace Replace this with the name of your bot with the username of the bot that you are requesting.
If you have not already created the user account for the bot, a consul will create it for you.

Next, replace Replace this with what the bot does (everything) with the activity(ies) you want your bot to perform.

Finally, replace Replace with this the reason for requesting the bot with your reason for requesting the bot,
as well as any technical specifications that would be useful information to action your request.

Please don't put anything in the Subject/headline field: It will be filled in automatically.

Remember that after 24 hours of being administrator and 10-20 edits, you can request bureaucrat!
